Please bring your photo ID, your COVID vaccine card (if you have one) and your insurance card (if you have one).  COVID-19 vaccines are free of charge.  If you have insurance, Medicaid or Medicare, we are allowed to bill a small administration fee to your insurance company to help us offset the costs of providing vaccine clinics.  If your insurance doesn't pay, we will NEVER balance bill you for a COVID-19 vaccine.  
CAR SHOTS:  If you are able to leave your home but unable to leave your car and come into our clinic due to a disability, we can bring your shot to your car.  If you need this service, please have the person who is accompanying you come into our clinic during our COVID vaccine hours and get your paperwork.  Then we will bring your shot to your car.  
HOMEBOUND COVID VACCINES:   If you have a disability and you are unable to leave your home to get a COVID-19 shot, AND you are a resident of Scioto County (outside of Portsmouth City), we may be able to bring a COVID-19 vaccine to you at home.  Please call us at 740-355-8358 option 1 (nursing) to request a homebound COVID vaccine.
