How to Order Certificates

Vital Statistics of Scioto County
The Vital Statistics of Scioto County Health Department maintains birth records from December 20, 1908 to present of births anywhere in OHIO.  Death records from 1931 to present (only if death occurred in Scioto County outside of Portsmouth City limits).  Costs are $25.00 per certified copy.  Scioto County Health Department in accordance with Scioto County Auditor Office accepts cash, money orders, cashier's and local checks made payable to Scioto County Health Department.  Records may be requested in person Monday-Friday from 8:30-4:15.  ​  You may also send a request for a birth or death record by mail. 
Please include;  the request form, method of payment  a phone number, and a stamped, self-addressed envelope.  Please do not send cash by mail.

Marriage and divorce records are NOT maintained by the Bureau of Vital Statistics.  Certified copies of marriage licenses and divorce decrees can only be obtained from the county where the event was recorded.

  • Marriage certificate copies can be obtained from the specific county probate court.  In Ohio each county probate court maintains their own online contact pages and information.
